EFEKTIVITAS KOMBINASI GYM BALL DAN AROMATERAPI LAVENDER TERHADAP MANAJEMEN NYERI PERSALINAN KALA I FASE AKTIF PADA IBU BERSALIN DI PRAKTIK MANDIRI BIDAN

Nyeri persalinan merupakan salah satu keluhan utama yang dialami hampir seluruh ibu bersalin, dengan prevalensi mencapai 85–90% pada kala I fase aktif. Manajemen nyeri non-farmakologi seperti Gym Ball (Birth Ball) dan aromaterapi lavender dinilai lebih aman, murah, dan minim efek samping dibandingkan metode farmakologi, serta dapat dikombinasikan untuk memberikan efek sinergis. Penelitian ini bertujuan menganalisis pengaruh Gym Ball dan kombinasi Gym Ball dengan aromaterapi lavender terhadap penurunan intensitas nyeri persalinan kala I fase aktif di Praktik Mandiri Bidan (PMB). Desain penelitian adalah quasi-experimental dengan pendekatan pretest-posttest two group design, melibatkan 30 ibu bersalin yang terbagi menjadi Kelompok 1 yang menerima intervensi Gym Ball berupa gerakan pelvic rocking (n=15) dan Kelompok 2 yang menerima kombinasi Gym Ball dan aromaterapi lavender secara simultan (n=15), masing-masing selama 30 menit. Intensitas nyeri diukur menggunakan Numeric Rating Scale (NRS) sebelum dan sesudah intervensi. Analisis data menggunakan uji Wilcoxon Signed-Rank Test dan Mann-Whitney U Test. Hasil penelitian menunjukkan rerata skor nyeri Kelompok 2 (kombinasi Gym Ball dan aromaterapi lavender) turun signifikan dari 7,40 menjadi 4,40 (p=0,001; r=1,00), sedangkan Kelompok 1 (Gym Ball tunggal) hanya turun dari 7,27 menjadi 6,27 (p=0,001). Uji Mann-Whitney U menunjukkan perbedaan penurunan nyeri yang sangat bermakna antar kelompok (U=0,000; p=0,001; r=0,95). Penelitian ini menyimpulkan bahwa kombinasi Gym Ball dan aromaterapi lavender lebih efektif dan bermakna secara klinis dalam menurunkan intensitas nyeri persalinan kala I fase aktif dibandingkan penerapan Gym Ball secara tunggal, sehingga direkomendasikan sebagai bagian dari standar operasional prosedur manajemen nyeri non-farmakologi multimoda di fasilitas pelayanan kebidanan primer.
Labor pain is one of the main complaints experienced by almost all mothers during childbirth, with a prevalence of 85–90% during the active phase of the first stage. Non-pharmacological pain management, such as the Gym Ball (Birth Ball) and lavender aromatherapy, is considered safer, cheaper, and has minimal side effects compared to pharmacological methods, and can be combined to provide a synergistic effect. This study aims to analyze the effect of the Gym Ball and the combination of Gym Ball with lavender aromatherapy on reducing labor pain intensity during the active phase of the first stage at Midwife Private Practice (PMB). The study design is quasi-experimental with a pretest-posttest two-group design, involving 30 mothers in labor divided into Group 1, which received the Gym Ball intervention with pelvic rocking movements (n=15), and Group 2, which received the combination of Gym Ball and lavender aromatherapy simultaneously (n=15), each for 30 minutes. Pain intensity was measured using the Numeric Rating Scale (NRS) before and after the intervention. Data analysis was performed using the Wilcoxon Signed-Rank Test and the Mann-Whitney U Test. The study results showed that the average pain score in Group 2 (combination of Gym Ball and lavender aromatherapy) dropped significantly from 7.40 to 4.40 (p=0.001; r=1.00), while Group 1 (Gym Ball alone) only decreased from 7.27 to 6.27 (p=0.001). The Mann-Whitney U test showed a highly significant difference in pain reduction between the groups (U=0.000; p=0.001; r=0.95). This study concludes that the combination of Gym Ball and lavender aromatherapy is more effective and clinically meaningful in reducing labor pain intensity during the first active phase compared to using the Gym Ball alone, and is therefore recommended as part of the standard operating procedures for multimodal non-pharmacological pain management in primary maternity care facilities.
