HUBUNGAN ULKUS DIABETIKUM DENGAN KEPERCAYAAN DIRI PADA PASIEN DIABETES MELLITUS DI KABUPATEN SITUBONDO

Ulkus diabetikum merupakan komplikasi kronis Diabetes Melitus yang tidak hanya berdampak pada kondisi fisik, tetapi juga aspek psikologis, salah satunya penurunan kepercayaan diri. Perubahan kondisi fisik akibat ulkus dapat menyebabkan pasien merasa minder, membatasi interaksi sosial, dan menurunkan keyakinan terhadap kemampuan dirinya. Penelitian ini bertujuan menganalisis hubungan ulkus diabetikum dengan kepercayaan diri pada pasien Diabetes Melitus di Kabupaten Situbondo. Penelitian ini menggunakan desain kuantitatif korelasional dengan pendekatan cross-sectional. Populasi berjumlah 104 pasien Diabetes Melitus dengan ulkus diabetikum, dengan sampel sebanyak 83 responden yang dipilih menggunakan teknik purposive sampling. Ulkus diabetikum diukur menggunakan klasifikasi Wagner dan kepercayaan diri menggunakan Rosenberg Self-Esteem Scale (RSES). Analisis data menggunakan uji Spearman's Rho dengan tingkat signifikansi α = 0,05. Mayoritas responden mengalami ulkus diabetikum grade 3 (54,2%) dan memiliki kepercayaan diri rendah (84,3%). Hasil uji Spearman's Rho menunjukkan nilai p = 0,047 dengan koefisien korelasi r = -0,218, yang menunjukkan hubungan signifikan dengan kekuatan lemah dan arah negatif. Semakin tinggi grade ulkus diabetikum, semakin rendah kepercayaan diri pasien. Perawat perlu memberikan asuhan yang komprehensif dengan memperhatikan aspek fisik dan psikologis melalui edukasi, dukungan emosional, serta peningkatan self-efficacy untuk meningkatkan kepercayaan diri pasien.
Diabetic foot ulcers are a chronic complication of Diabetes Mellitus that affect not only physical health but also psychological well-being, particularly self-confidence. Physical changes caused by diabetic foot ulcers may lead patients to feel inferior, limit their social interactions, and reduce their confidence in their own abilities. This study aimed to analyze the relationship between diabetic foot ulcers and self-confidence among patients with Diabetes Mellitus in Situbondo Regency. This study employed a quantitative correlational design with a cross-sectional approach. The population consisted of 104 patients with Diabetes Mellitus who had diabetic foot ulcers, and a sample of 83 respondents was selected using purposive sampling. Diabetic foot ulcers were assessed using the Wagner Classification, while self-confidence was measured using the Rosenberg Self-Esteem Scale (RSES). Data were analyzed using Spearman's Rho test with a significance level of α = 0.05. The majority of respondents had Grade 3 diabetic foot ulcers (54.2%) and low self-confidence (84.3%). The results of Spearman's Rho test showed a p-value of 0.047 with a correlation coefficient (r) of -0.218, indicating a statistically significant weak negative correlation. The higher the severity of diabetic foot ulcers, the lower the patients' self-confidence. Nurses are expected to provide comprehensive nursing care by addressing both physical and psychological aspects through health education, emotional support, and self-efficacy enhancement programs to improve patients' self-confidence.
