ASUHAN KEBIDANAN KOMPREHENSIF PADA NY. “Y” DI PMB Hj.AZIA NOFA,STr.,Keb.Bd KECAMATAN IV KOTO KABUPATEN AGAM TAHUN 2024

Abstrak

Asuhan Kebidanan Komprehensif adalah asuhan yang diberikan secara berkesinambungan kepada ibu selama kehamilan, persalinan, nifas, bayi baru lahir. Tujuan untuk menurunkan Angka Kematian Ibu dan Angka Kematian Bayi supaya kesehatan ibu dan bayi terus meningkat. Tujuan dilakukan studi kasus ini untuk memberikan asuhan komprehensif pada ibu menggunakan pendokumentasian Tujuh Langkah Varney dan SOAP. Pengkajian ini dilakukan sejak bulan Februari sampai bulan Mei 2024 dengan metode pengumpulan data. Asuhan kehamilan dilakukan kunjungan sebanyak 3 kali pada usia kehamilan 29-30 minggu, 33-34 minggu dan 38-39 minggu dengan HPHT 28 Juli 2023 dan TP  05 Mei 2024. Selama kunjungan didapatkan pemeriksaan keadaan umum ibu baik. Asuhan persalinan dilakukan pada tanggal 04 Mei 2024 jam 01.00 WIB ibu datang ke rumah bidan bersama suami dengan keluhan perut mules dan keluar lendir bercampur darah sejak pukul 22.00 WIB, pukul 01.00 dilakukan pemeriksaan dalam VT 4 cm, pukul 03.00 dilakukan pemeriksaan dalam VT 8 cm, pukul 03.30 WIB pembukaan lengkap air ketuban pecah spontan dan dilakukan pimpinan persalinan. Proses persalinan ibu bersalin secara normal dengan 60 langkah APN, bayi lahir spontan, menangis kuat, tonus otot aktif, jenis kelamin Laki laki, BBL 3000 gram, PB 46 cm segera dilakukan perawatan bayi baru lahir. Asuhan masa nifas dan bayi baru lahir berjalan dengan lancar dan tidak ada komplikasi pada ibu maupun bayinya.

Comprehensive Midwifery Care is care provided on an ongoing basis to mothers during pregnancy, childbirth, postpartum, and newborns. The aim is to reduce the maternal mortality rate and infant mortality rate so that the health of mothers and babies continues to improve. The aim of this case study is to provide comprehensive care to mothers using Varney's Seven Steps documentation and SOAP. This study was carried out from February to May 2024 using data collection methods. Pregnancy care was visited 3 times at gestation ages 29-30 weeks, 33-34 weeks and 38-39 weeks with HPHT 28 July 2023 and TP 05 May 2024. During the visit, the mother's general condition was examined well. Childbirth care was carried out on 04 May 2024 at 01.00 WIB. The mother came to the midwife's house with her husband with complaints of a stomach ache and mucus mixed with blood coming out since 22.00 WIB, at 01.00 an examination was carried out in a VT of 4 cm, at 03.00 an examination was carried out in a VT of 8 cm, At 03.30 WIB the amniotic fluid was completely opened and the birth control was carried out. The birth process of the mother gave birth normally with 60 steps of APN, the baby was born spontaneously, cried strongly, active muscle tone, male gender, BBL 3000 grams, PB 46 cm and immediate care for the newborn was carried out. Postpartum and newborn care went smoothly. smoothly and there were no complications for the mother or baby.
