PERBANDINGAN VARIASI WAKTU FIKSASI JARINGAN HISTOLOGI MENGGUNAKAN 10% NEUTRAL BUFFERED FORMALIN PADA PEWARNAAN HEMATOXYLIN EOSIN: SYSTEMATIC LITERATURE REVIEW

Pemeriksaan histopatologi merupakan metode diagnostik utama untuk menilai morfologi jaringan melalui tahapan histoteknik, dengan fiksasi sebagai langkah awal yang paling krusial. Fiksasi mengawetkan struktur sel menggunakan larutan Neutral Buffered Formalin (NBF) 10% yang bekerja membentuk ikatan silang antarprotein guna mencegah autolisis. Penelitian ini bertujuan untuk mengkaji dan membandingkan pengaruh variasi waktu fiksasi NBF 10% terhadap kualitas hasil pewarnaan Hematoxylin Eosin (HE). Manfaat dari kajian ini adalah memberikan acuan ilmiah berbasis bukti bagi tenaga laboratorium patologi anatomi dalam menetapkan durasi fiksasi yang optimal guna meningkatkan akurasi diagnosis medis. Metode penelitian menggunakan pendekatan Systematic Literature Review (SLR) berbasis panduan PRISMA dengan menelaah artikel jurnal primer bereputasi terbitan tahun 2016–2026. Hasil sintesis dari studi terdaftar menunjukkan bahwa fiksasi berdurasi sangat singkat (1–5 jam) pada jaringan prostat menghasilkan gambaran mikroskopis yang belum optimal, di mana fiksasi 1 jam memberikan hasil terburuk (70% sampel tidak jelas) dan fiksasi 5 jam memberikan hasil terbaik (80% sampel jelas), meskipun secara statistik tidak signifikan (p = 0,074). Sebaliknya, penelitian pada jaringan mioma uteri dengan variasi durasi lebih panjang menunjukkan perbedaan kualitas yang signifikan secara statistik (p = 0,006). Fiksasi selama 24 jam dan 48 jam menghasilkan morfologi sel, kejelasan inti, serta kontras warna yang jauh lebih unggul dibandingkan fiksasi 12 jam (p < 0,05), tanpa ada perbedaan signifikan antara kelompok 24 jam dan 48 jam (p = 1,000). Kesimpulannya, waktu fiksasi NBF 10% yang terlalu singkat (<12 jam) dapat menurunkan kualitas mikroskopis preparat, sedangkan durasi fiksasi ideal berkisar antara 24 hingga 48 jam untuk menghasilkan preparat HE bermutu tinggi yang mendukung ketepatan diagnosis.
Histopathological examination is the primary diagnostic method for assessing tissue morphology through histotechnical procedures, with fixation serving as the most crucial initial step. Fixation preserves cellular structures using a 10% Neutral Buffered Formalin (NBF) solution, which works by forming cross-links between proteins to prevent autolysis. This study aims to examine and compare the effects of varying fixation times with 10% NBF on the quality of Hematoxylin-Eosin (HE) staining results. The benefit of this study is to provide evidence-based scientific guidance for anatomical pathology laboratory personnel in determining the optimal fixation duration to improve the accuracy of medical diagnoses. The research method employed a Systematic Literature Review (SLR) approach based on the PRISMA guidelines, analyzing reputable primary journal articles published between 2016 and 2026. The results of the synthesis of the included studies indicate that very short fixation durations (1–5 hours) in prostate tissue yield suboptimal microscopic images, with 1-hour fixation yielding the worst results (70% of samples were unclear) and 5-hour fixation yielding the best results (80% of samples were clear), although this difference was not statistically significant (p = 0.074) . In contrast, a study on uterine fibroid tissue with longer fixation durations showed statistically significant differences in quality (p = 0.006). Fixation for 24 hours and 48 hours produced significantly superior cell morphology, nuclear clarity, and color contrast compared to 12-hour fixation (p < 0.05), with no significant difference between the 24-hour and 48-hour groups (p = 1.000). In conclusion, a fixation time with 10% NBF that is too short (<12 hours) can reduce the microscopic quality of the specimen, whereas the ideal fixation duration ranges from 24 to 48 hours to produce high-quality HE-stained specimens that support accurate diagnosis.
